I will take responsibility for all other school or training expenses which are in excess of the US $1,000 grant should I be the recipient of the Volunteer Leadership Scholarship. I grant the United Way of Defiance County permission to use my name and photograph in the literature designed to promote United Way of Defiance County and the Community Engagement Center High School Volunteer Program. I certify that all of the information contained in my application form is accurate to the best of my knowledge. I consent to the hard copy publication or electronic publication by United Way of Defiance County of material in my application.
